How to remove yourself from LexisNexis

What you'll need
- Your full name, street address, city, state, ZIP, and date of birth
- Optionally an SSN or driver’s licence number + state (improves verification)
- No email is strictly required — a phone option is offered
Steps
- Open the consumer request form
Go to consumer.risk.lexisnexis.com/request. This is LexisNexis’s own consumer portal for accessing and requesting suppression of your data.
- Fill in the required identity fields
Enter your first and last name, street address, city, state, ZIP and date of birth. Optionally add an SSN or driver’s licence number and state — LexisNexis recommends one of these for faster, more reliable verification.
- Select the request type and submit
Choose the appropriate request (for example, your consumer disclosure), add a delivery address if it differs from your residence, and submit the form. If they can’t process it online, LexisNexis follows up by U.S. Mail.
